La Big Sister tendrá un aspecto alto, delgado, tipo traje de buzo, con su peculiar escafandra con un solo ojo rojo, también será el enemigo mas rápido. Los Splicers han mutado para peor y es posible que salga algún personaje del BioShock original. El jugador podrá llevar un taladro, una pistola de remaches y plásmidos (de momento). Se podrá adoptar, cosechar o rescatar a las Little Sisters. Al inicio del juego sólo se podrán cosechar o adoptar, rescatar se volverá una opción en algún momento del juego. Nuevamente el juego tendrá un final bueno y uno malo a función de las decisiones del jugador
